Registration fees for are as follows: $445.00 until August 8
and $495.00 after that.
The registration fee includes continental breakfast, coffee breaks, lunch with the speakers,CME/CEU credit certificate, and conference materials .
No payment is due at time of application.

Cancellation Policy
Space is limited and available on a first-come, first-served
basis. Refund, less a $50 processing fee, will be made if cancellation
is received no later than August 6, 2008. No refunds will be granted
after August 8, 2008. Substitutions, however, are welcome.
